US Standard Invoice Template: Common Fields and a Sample Layout

September 2026 ยท 6 min read

Many US businesses use a familiar set of invoice fields, but the right document can depend on the transaction, customer agreement, industry and applicable rules. The outline below is a practical starting point, not a legal or tax checklist for every business.

The layout that US clients recognise

  • Your company block โ€” legal business name, street address, city/state/ZIP, email and phone. If you have an EIN, most corporate clients like to see it; it's optional but common.
  • Bill To block โ€” client name, address, and email when they want it for their records.
  • Invoice number and dates โ€” a reference number, issue date, and payment date or terms agreed for the transaction.
  • An item table โ€” description, quantity, unit price and line total. Clear descriptions can make charges easier to review; tax treatment and record-keeping obligations depend on the transaction and location.
  • Totals โ€” subtotal, any tax, and a clearly bolded total. Clients scan straight for this number.
  • Notes โ€” payment instructions (how to pay, where to send it) and a thank-you line. Sounds small; improves payment speed more than you'd think.

Sales tax: the part that's actually state-by-state

U.S. sales and use tax is administered by states and, in some places, local authorities. Whether a transaction is taxable and whether you must register or collect tax can depend on the seller's activities, the product or service, sourcing rules and other facts. Verify the rules with the relevant tax authority before applying a rate; a zero in the form is not a taxability determination.

EIN vs SSN on invoices

An EIN and an SSN are different taxpayer identifiers. An ordinary invoice generally should not ask for a full SSN; do not add it to a document that may be emailed or shared. If a client requests taxpayer information for a separate tax form, confirm what is required and use a secure channel.

Frequently asked questions

Does every US business use the same invoice format? No single layout covers every transaction. Agreements, industry requirements and applicable tax rules may call for additional information; verify what applies to your situation.

Should I show sales tax separately? If tax applies, present it as required by the relevant state or local rules. This general guide cannot determine whether a particular sale is taxable.

What if a client asks for a W-9? A client may request a completed W-9 separately from an invoice. Confirm the request and use an appropriate secure method to share taxpayer information.
